The candidates who qualify for viva-voce test submit one (01) set of following documents duly verified by the concerned Board/University and attested by Gazetted Officer within Fifteen (15) days of the Notice from BPSC.
1- Five (5) Passport size latest photographs (Female candidates also)
2- Computerized National 1D card
3- Original Treasury Receipt (Green Colour)
4- Matric DMC & Certificate.
5- Intermediate DMC & Certificate.
6- Pharm-D Transcription and Degree
7- Local/Domicile Certificate (duly verified by the competent authority).
8- One Provisional & Character Certificate (Last attended Educational Institution).
9- Two Character Certificates from different Gazetted Officer.
10- Photocopy of filled one page application form.
11- Experience Certificate (where required) showing nature of job/ detailed job description issued
by an authorized officer of the Department concerned.
12- Departmental Permission Certificate (in case of government servant) etc.
